I AUTHORIZE THE USE OF MY IMAGE AND PERSONAL DATA
I acknowledge that the activities of the 1st QuaDRA Workshop – Quantum Devices for Research and Applications may be recorded or photographed, and I hereby AUTHORIZE the use of my image in any and all materials, including photographs, videos, and documents, for promotional, educational, scientific, and institutional purposes by the Instituto de Física Gleb Wataghin (IFGW) at Universidade Estadual de Campinas (Unicamp), as well as by the QuaDRA initiative and its partner institutions.
This authorization is granted free of charge and covers the use of the aforementioned image in both national and international territories, in all forms and media, including but not limited to:
- (I) printed materials in general (brochures, catalogs, proceedings, reports, etc.);
- (II) presentation materials;
- (III) websites and webpages;
- (IV) posters and banners;
- (V) electronic and social media (digital displays, television, radio programs, blogs, Facebook, Instagram, LinkedIn, YouTube, and similar platforms); and
- (VI) general publicity, outreach, scientific dissemination, and reporting activities related to the Workshop and the QuaDRA initiative.
As this authorization reflects my free and informed consent, I declare that I authorize the use described above and waive any claim related to image rights or any other associated rights.
I agree to provide my personal data (full name, email address, CPF or passport number, and institutional affiliation) for the purpose of completing my registration for the 1st QuaDRA Workshop – Quantum Devices for Research and Applications. I acknowledge that Unicamp will process my personal data solely for the organization, administration, communication, attendance management, certificate issuance, and reporting activities related to the Workshop, in accordance with the Brazilian General Data Protection Law (Law No. 13,709 of August 14, 2018) and the University's Privacy Policy (CAD Resolution A-03/2020).
